• Level 4 or higher do not honor memberships
• Gratuity not included
• Service cancellations require 24 hour notice / charges may apply for not showing to appointment or canceling within 24 hours of appointed time
• Membership cancellations require 30 days written notice
• Membership suspended for the month if credit card on file for billing is declined after 2nd attempt
• 6 month agreement required for all membership
